Application rejected and no help is provided

My new account application form got rejected and shows this problem:
Name Declaration and additional ID proof Required
( if Name is already Mention in KRA then correction and client signature on KRA page is required)

I got a message with a contact number to help me but it’s of no use. What should I do?
